Eligibility: Must have an initial consultation prior to establish care. Members must be 18+ Services subject to provider approval and health screening.
Therapies: There are no limits of when you can use your IV’s or HBOT sessions, Meyer’s IV can be changed to high dose Vit C or a custom IV bag, but you will be responsible for paying the difference. Should any new conditions arise or significant concerns develop, a separate consultation will be necessary, and associated fees will apply. Lab orders include free order; you are responsible for draw fees and cost of labs.
Payment: Membership fees are due in full at the start of each term. Credit Card needs to be on file. Missed payments may suspend access until resolved. Non-refundable after activation. We accept credit, debit, HSA and Care Credit (with 6 months 0% financing!)
Communication: Provider communication is for non-emergent care only. For emergencies, call 911.
Cancellation: Early cancellation forfeits unused benefits. Renewals may be declined with 14-day written notice prior to next term. Benefits are non- transferable.
Discount Exclusions: Cannot combine with other promotions, packages, or gift certificate purchases.
Privacy: All personal health information is protected under HIPAA. The Flow Co. IV+02 does not share member data without consent.
